Each year, we ask that parents and guardians review and update their student's information in our Family Access Qmlativ database. This information allows FPS and your student's school to stay connected with you and access vital information about your child's health and safety.

Annual Student Information Update Tutorial

STEP 1: Log in to Qmlativ. If you don't know your password, please click the link highlighted in yellow on the login page to reset your password.

STEP 2: Once you have logged in, select one of the three update tiles

STEP 3: Click the 'Start' button to begin 

STEP 4: Review and update emergency contact information.

STEP 5: Enter information about translation and interpretation services

STEP 6: Review all information in form and submit.

STEP 7: Return to home page, select next tile to update

STEP 8: Review and update information about life-threatening conditions.

STEP 9: Review and update information for consent of exchange of confidential information.

STEP 10: Review information and submit form.

STEP 11: Return to home page, select next tile to update

STEP 12: Review Attendance Policies and Procedures

STEP 13: Review Request to Restrict Release of Information

STEP 14: Review information and submit form.
